1)    Change of Name

Name change deed from solicitor

 

2)    Change of Name to husband’s name (female applicants)

Marriage certificate

 

3)    Change of address

Proof of new address - Any authentic document containing the name and new address of the applicant (driving license, utility bills or bank statement)
