Changes in Passport

§         A change of address in your passport will require

                                i.      Completed application form C

                               ii.      Proof of new address like utility bill or bank statement

 

§         A change in the date of birth on your passport will require

                                i.      Completed application form C

                               ii.      Original Birth Certificate along with its photocopy

Please note that if Birth Certificate is issued from Pakistan, it needs to be first attested from Ministry of Foreign Affairs in Pakistan

                             iii.      Original ID Card along with its photocopy

 

§         For change in name on your passport you need to provide

                                i.      Completed application form C

                               ii.      A deed poll from a solicitor specifying the name to be changed

                             iii.      original Birth Certificate along with its photocopy

Please note that if Birth Certificate is issued from Pakistan, it needs to be first attested from Ministry of Foreign Affairs in Pakistan

                            iv.      Original ID Card along with its photocopy

For all type of changes in passport is fee is £ 8 + 2 (Bank charges)
